In completing this form, you consent to being contacted by The Acorn Nursery in relation to this application, via the details you have provided. If your child is offered a place at The Acorn Nursery, the information you provide on this form will be used to create their school record. Unsuccessful applications will be held for up to 6 months after a decision has been made and then securely disposed of.

I understand and agree that bullying is not tolerated in this school and will abide by the established rules and guidelines. I understand that if I fail to follow this agreement, I may be subject to appropriate disciplinary action.
