Form 1 of 3 – Child & Family Registration

Welcome to the BAF Family

This first form captures your child's essential details, your contact information, and your membership selection. It is the starting point of your child's BAF journey, please complete it at your own pace.

A warm note from our team: Forms 2 and 3 will be sent to you over the coming days. Form 2 covers your child’s health and medical information. Form 3 covers consents and your parent agreement.
